What must be reported

Hazards observed

Injury or illness

Report immediately and trigger first aid and escalation.

Protect the area

Isolate risk where safe, stop exposure.

Notify

Tell your supervisor and host safety lead now.

Document

Capture time, location, people, equipment and evidence.

Follow up

Keep contact with the chain until controls close.

Urgent incidents

Call emergency services for immediate danger, then complete reporting and first aid escalation.
